I the undersigned, do hereby irrevocably and unconditionally undertake to be personally liable for any and all transactions and/or dealings conducted and/or undertaken by me personally or by or on behalf of any of my businesses as a member of Dubai Diamond Exchange. I confirm that all information provided herein is true and accurate and I undertake to update Dubai Diamond Exchange promptly with any changes in respect thereof. I agree to abide by all rules, regulations and by-laws issued by Dubai Diamond Exchange, Dubai Multi Commodities Centre Authority and rules of the World Federation of Diamond Bourses, as shall be amended from time to time.

1. Attested passport copy certified by a bank or authorized public notary*

2. Business Experience Form*

3. Proof of representative's residential physical address ( i.e. a copy of utility bill or bank statement)*

4. One passport size photographs*
